  • Schedule: Minimum 4 full days/month (preferably Mon–Fri); half-days allowed after minimum met

  • Assignment Length: 1 year, with potential extension
  • Estimated Weekly Pay: $480-533
  • Travel: Daily travel to patient homes (up to 120 miles/day depending on territory)

Nurse Practitioner Qualifications

  • Active, unrestricted Nurse Practitioner license in Kentucky
  • Board Certification (ANCC, AANP, or AACN) as AGNP, FNP, ENP, or ACNP
  • Minimum 1 year of independent Nurse Practitioner experience
  • Experience with in-home healthcare or mobile clinical support preferred
  • Comfortable traveling alone to patient homes in rural, urban, and underserved areas
  • Strong adaptability and flexibility with changing schedules and routes
  • Excellent communication and patient care skills
  • Must provide own patient bag with basic medical tools
  • Professional attire or dark-colored scrubs required (no lab coat needed)

Nurse Practitioner Responsibilities

  • Conduct annual in-home Health Risk Assessments for geriatric patients (1:1 visits, no follow-up required)
  • Perform comprehensive physical exams, medication reviews, and assess social/environmental factors
  • Complete basic labs (e.g., A1C, fit tests – no blood draws)
  • Document and submit assessments using a provided tablet (same-day submission required)
  • Efficiently travel within assigned counties, adapting to daily route changes
  • Maintain high quality scores and adhere to clinical best practices
  • Communicate effectively with patients, families, and the clinical support team
  • Manage logistical unpredictability, such as no-shows or last-minute cancellations
